如學校（小學或中學）在處理學生朋輩間的衝突或校園欺凌的問題時，需要校外機構進一步為學生提供專項的諮詢或

個案輔導，學校社工或教師請填寫以下《個案轉介表》至香港遊樂場協會跟進。

Case Referral Form

If specified consultation services or case referrals to the external organisation are required in the course of handling peer

conflicts among students or school bullying, school social worker(s) or teachers of the primary or secondary schools can

refer the case(s) to HKPA through the submission of the Case Referral Form for follow-up.
